Request for term time absence
Please use this form to request any absences (apart from medical appointments) during term-time e.g. holidays, visiting relatives, etc.
Your request will be passed to the relevant person(s) for authorisation and the school will contact you as soon as possible with the outcome.
Alternatively, the office can provide you with a printed copy
Medical appointment during the school day
Please use this form to request to take your child(ren) for a medical, hospital, dentist, optician appointment during the school day. Teachers will then be informed of your request.
Please collect your child from the school office.
Parental consent for processing personal data
We use consent as our basis to process your child’s personal data in certain circumstances. This form also details the activities in which we use consent.
